Getting the current or session quote on admin is different that on front office.
I be sure to get the right one, use this :
if(Mage::app()->getStore()->isAdmin()) {
$quote = Mage::getSingleton('adminhtml/session_quote')->getQuote();
}else{
$quote = Mage::getSingleton('checkout/session')->getQuote();
}

To know if you are in the adminhtml or in another store, you can simply do :
if(Mage::app()->getStore()->isAdmin()){
//I'm in the admin
}
You made a form in adminhtml with upload file but in the controller the $_FILES var is hopelessly empty ?
You probably forgot to add “‘enctype’ => ‘multipart/form-data’,” on your form declaration
